Home Health Care is Personalized Care

At Butte Home Health, we know people feel better when they’re cared for in the privacy and comfort of their home. Whether it’s chronic illness, an acute medical problem, injury or disability – we are there every step of the way to ensure our patients receive the best possible medical care delivered by caring professionals. We are Medicare/Medi-Cal certified and have been caring for the members of our community since 1984.

  • Home health benefits bring skilled nursing, therapy, or even medical social work visits to your home that help you recover, stay safe, and remain independent. All services are done under the supervision of your doctor.
  • Home health can help you transition home after a hospital stay or after a stay at a rehabilitation facility. Home health can also be ordered by your doctor if your health condition or medications change. Our caring professionals visit intermittently (often two or three times a week) and focus on helping you remain independent at home. They will teach you about your condition, help you understand your medications and ensure you take them at the right time and in the right amounts, working to help you with your nutrition needs, keeping you safe from falls risks, and get back to doing your normal activities around your home.
  • Home health is highly skilled, delivered by experienced professionals, and provides a personal plan of care to get you going again, safely and independently while keeping you where you want to be most – in the comfort of your home.

Home Health uses a team approach that includes coordinated care designed to maximize independence at home. Depending what your doctor orders, Butte Home Health team members can provide:

Home Health Care

Home Health Nurse

Registered Nurses and Licensed Vocational Nurses can provide the following services:

  • Skilled assessments
  • Instruction and monitoring of medications and disease process
  • Symptom control
  • Instruction and monitoring of treatments (i.e: wound care)
  • Injectable medications
  • Nutrition and special diets
  • IV medications
  • Pain management
  • Foley catheter management

Therapy

Physical Therapy

Develops and restores function of limbs and muscles. A hands-on approach to enhance healing and decrease pain. Instructs patients and family in rehabilitative activities to achieve maximum independence

The following services can be provided:

  • Skilled evaluation
  • Therapeutic exercise program
  • Pain management
  • Mobility training (bed mobility, transfer training, gait training)
  • Massage
  • Ultrasound
  • Instruction in energy conservation

Occupational Therapy

The following services can be provided:

  • Skilled evaluation
  • Training and exercise
  • Developing a program to promote independence, including: Dressing, Personal Hygiene, Meal Preparation, and Home Maintenance
  • Identification and training in the use of adaptive equipment to promote independence with activities of daily living

Speech Therapy

Provided by a Licensed Speech Pathologist to help regain or maintain physical or mental functions.

Home Health Care

Additional Services

Medical Social Worker

  • Assist with long-term plans, obtaining financial aid and community resources.
  • Assessment of social and emotional factors impacting the patient’s care.
  • Counseling for the patient and caregiver.

Home Health Aides

  • Home Health Aides provide personal care for the patient, including; Showering, Skin Care, Shampooing, and Bed Positioning.
  • Therapeutic exercises as prescribed by a licensed therapist.
  • Assistance in maintaining a clean, healthy home environment for the patient.

Call Us When:

  • You or a loved one is in the hospital or rehabilitation care facility and want to go home.
  • You’re feeling overwhelmed by the demands of providing care for a loved one.
  • You or a loved one want to remain at home but need help to do so.
  • You’ve hired a new caregiver that needs instruction and training to provide care in your home.

Initiating:

  • A patient, caregiver, family member, physician, or concerned person may request services by calling Butte Home Health.
  • Following a request for home health care, the patient’s physician will be contacted for approval of services.
  • You can initiate services by calling (530) 895-0462 during office hours: 8:30 AM to 5:30 PM, 7 days a week
